  • The complete pickup address and the destination clinic or facility name.

  • The passenger's mobility status (Do they require a specialized wheelchair van, or are they walking/ambulatory?).

  • Any specific transfer requirements, such as navigating exterior stairs or requiring arm-thru-arm balance assistance.
